NATURE OF WORK:

  • The case Manager provides intake and ongoing assessments of infants and toddlers eligible for Early Intervention Services under California Early Start program mandates. 
  • Assists eligible children and their families with the coordination of other resources, in the development of an Individualized Family Service Plan participates in periodic reviews of the IFSP every six months or more frequently if conditions warrant, and takes an active role in transition social services for toddlers. 
  • The Early Intervention Service Coordinator provides social service coordination to assist families in accessing services necessary to enhance the development of the child and the capacity of the family to meet the needs of the child. 
  • May participate in educational and prevention outreach activities and serve as liaison to other community organizations that work with populations at high risk for having a developmental disability.

MINIMUM EDUCATION AND EXPERIENCE:

  • The Early Intervention Case Manager must have a Master's Degree in Social Work or a related field from an accredited university, be a credentialed Speech Pathologist or Nurse with a PHN or PNP certification, and have three years of experience in working with children in early intervention agencies and resources.
  • A Bachelor's Degree and three years experience in a professional position at NBRC or another California Regional Center with a child case carrying responsibility can be substituted for the above.
  • Knowledge of the Spanish Language is desirable. Minimum qualifications may be waived depending on the agency’s need for specialized skills (such as language abilities).

SPECIAL REQUIREMENTS:

  • Possession of a valid California driver’s license, and minimum legal automobile liability insurance required; 
  • The ability to provide personal automobile transportation during working hours and after-hours emergencies or access to another form of reliable transportation that meets these minimum legal requirements.

A note regarding NBRC’s expectations regarding remote, hybrid, and home-based work… 

All employment at NBRC requires face-to-face, in-person interactions with NBRC clients, family members, community, and co-workers. Candidates for employment must live within commute distance to their preferred NBRC office, as well as the geographical location of their assigned clients. While 100% remote work is not available to NBRC employees, a hybrid work arrangement with a blend of in-office and home-based work may be available for new employees after successfully demonstrating achievement of performance expectations following at least 3 months of in-office work.
